Make an Appointment

Patients are encouraged to call for appointments as far in advance as possible. If you request or are referred to a particular surgeon, we will make every attempt to accommodate you, otherwise, you will be scheduled with the surgeon on call. If time allows before your appointment, a registration packet can be downloaded here or can be mailed it to you if you prefer. This will streamline the process when you arrive at our office.

Remember to bring any x-rays and reports that might be pertinent to your visit.
